New Patient Forms
We are always welcoming new patients. Please download our new patient forms, print them out, and bring the completed forms to our office to expedite the paperwork at your first visit for your convenience. Adobe Acrobat Reader is needed to download these forms.
For those who are joining our family of patients, it might seem out of the ordinary to be scheduled for a 90-minute new patient visit with one of our doctors and a hygienist. This time is reserved especially for you to ensure that you receive a thorough examination and consultation. We look forward to getting to know you and your family, and to talking with you about your oral health and expectations for your smile. Our goal is not only to take care of our patients’ dental needs, but also to get to know each person by name and treat them like we would want our own family to be treated.
Scheduling
We strive to run on time because we understand that you are busy and your time is valuable. Our doctors only deviate from their schedule in the event of dental emergencies that need to be taken care of immediately, and we ask you to arrive for your appointments on time as well.
Once we have decided on a treatment plan and have worked out financial arrangements, we will do everything we can to proceed with your dental treatment on a schedule that works for you, whether that be soon or at a slower pace.
Please call at least two business days in advance for changed appointments. Missed appointments without this notification, or repeated cancellations, will incur cancellation fees. We want to work with you to schedule convenient appointment times for your visits to our office.
